Temporal bone fracture (otic capsule sparing)

Presentation

Intractable seizures. Head trauma following recent seizure-like activity.

Patient Data

Age: 20 years
Gender: Male

There is opacification of the right middle ear, mastoid air cells, and opacification right external auditory canal.

There is a nondisplaced longitudinally oriented fracture through the mastoid air cells, but spares the otic capsule. The carotid canal is intact. The ossicular chain is intact with the stapes footplate in the oval window. The round window niche is clear.

Case Discussion

This is a case of an otic sparing right temporal bone fracture.

The patient was admitted to the epilepsy monitoring unit where it was found that he had a previously undiagnosed frontal lobe epilepsy. Otolaryngology evaluated the patient for the temporal bone fracture. A non-operative treatment was pursued. The patient was scheduled for an outpatient audiogram.
