Presentation
Potential kidney donor. Incidental finding on renal CT angiography.
Patient Data
The right kidney shows a single renal artery
The left kidney shows triple accessory renal arteries arising from the lateral aspect of the abdominal aorta:
the upper renal artery supplies the upper pole and hilum
the middle renal artery supplies the renal hilum
the lower renal artery supplies the inferior renal pole
The CT urography shows normal collecting systems, ureters, and bladder.
Case Discussion
Features of accessory renal arteries with circumaortic left renal vein (incidental findings).
Accessory renal arteries are a common variant of the renal arteries. They are present in ~25% (range 20-30%) of the population and bilateral in ~10%. Triple renal arteries are seen in ~2.5% (range 1-4%) of the population.
Circumaortic left renal vein, also known as circumaortic renal collar is an anomaly of the left renal vein when a supernumerary or accessory left renal vein passes posterior to the aorta, apart from the normal renal vein passing anterior to the aorta.
CT angiography plays a key role in the preoperative evaluation of potential kidney donors. The main goal of this evaluation is to depict the renal vasculature (arterial and venous) and its variants and reduce the risks of complications.
