Presentation

Falling towards the right side with vertigo.

Patient Data

Age: 35 years
Gender: Male

The MRI sequences demonstrate a focal area in the right lateral medulla oblongata of low T1, high T2, and FLAIR with restricted diffusion in keeping with an acute focal infarct. The MRA shows complete occlusion of the right vertebral artery.

MRI features are most consistent with lateral medullary syndrome (LMS) or Wallenberg syndrome

The Wallenberg syndrome is most commonly due to an occlusion of the intracranial portion of the vertebral artery followed by PICA and its branches. The main risk factors are hypertension, followed by smoking and diabetes mellitus.
