Axial skeleton and proximal tubular long bones initially as this is the distribution of red-marrow in the older patient. Tumour spreads and crowds out normal marrow cells causing anaemia, thrombocytopaenia and a deficiency of normal white blood cells.
This patient went on to have a CT guided biopsy and the diagnosis of multiple myeloma was confirmed.
