Cytotoxic lesion of the corpus callosum associated with idiopathic intracranial hypertension

Discussion:

This patient presented with severe headaches associated with blurred vision, nausea and photophobia. MRI of the brain showed a cytotoxic lesion of the corpus callosum. No clear MR signs of idiopathic intracranial hypertension apart from questionable minimally prominent peri optic CSF spaces.

Wide investigations including lab were done and all were normal except for the fundus examination that showed grade II papilledema.

MRV was done, showing bilateral stenotic segments of the transverse venous sinuses suggesting benign intracranial hypertension.

The patient received Acetazolamide with marked improvement in her symptoms.

Follow-up fundus examination revealed resolved papilledema.

Follow-up MR revealed a complete resolution of the corpus callosum lesion.
