Fork sign (central sulcus)

The fork sign is a radiological sign that may help identify the central sulcus on neuroimaging 1. This sign is observed on sagittal planes containing the insular cortex and becomes less distinct at the level of the putamen 1.

It is based on recognising a tuning fork-shaped configuration of the gyral continuum that bridges the precentral and middle frontal gyri 1. The precentral gyrus forms the caudal branch of the fork sign, while its root lies directly above or points to the caudal short insular gyrus 1. The middle frontal gyrus constitutes the anterior branch 1.
