>75% fibre disruption (defect measuring up to 2 cm transverse and 4-5 cm craniocaudal) of the distal muscle belly extending to the distal myotendinous junction with torn fibres folded distally; the distal free tendon is intact and attached to the proximal humerus but is mildly high signal and oedematous. Adjacent fluid and haematoma.
