Central bronchiectasis, mucous plugging, high attenuation mucus, mosaic perfusion, air trapping on expiration.
What are the criteria for ABPA?
Both of the following: Type 1 Aspergillus skin test positivity/elevated IgE levels against A. fumigatus, elevated total IgE levels > 1,000 IU/mL, (can be < 1,000 IU/mL if all other criteria met), and 2 of: IgG antibodies against A. fumigatus or precipitating antibodies, pulmonary opacities on chest radiograph consistent with ABPA, eosinophils > 500/μL while not on corticosteroids.
Central varicose bronchiectasis in the right upper lobe, the medial segment of the middle lobe, the superior and posterior segments of the right lower lobe, the inferior lingular segment and the posterior and lateral segments of the left lower lobe. Hyperattenuating (+120HU) mucous plugging in the anterior and posterior segments of the right upper lobe and the superior and posterior segments of the right lower lobe and distal tree-in-bud opacities.
