The thoracic spine normally becomes increasingly lucent from superior to inferior. The "spine sign" occurs when consolidation is superimposed on the spine on the lateral view with loss of the normal increased lucency of the lower thoracic spine. Compare the appearance to the normal chest X-ray obtained 18 months prior.
