Detached peripheral capsular attachment of the posterior horn lateral meniscus (but still attached posteromedially to the tibia). The loose posterior horn has flipped anteriorly such that it now sits adjacent to the anterior horn (so-called double anterior horn or double delta signs). Note the there is now a gap or bare spot posteriorly where the posterior horn previously sat.
