A young male patient with neglected osteomyelitis of the 4^th metatarsal now describes swelling of the foot with a low-grade fever. CT shows an abnormal expansion of the 4^th metatarsal medullary cavity with thickening of the cortex as well as small lucent areas and cortical destruction near its base. CT findings suggest possible chronic osteomyelitis.
