The uterus is moderately enlarged with thickening of the junctional zone mainly posterior (thickness=12-16 mm) in keeping with adenomyosis.
There is a large right ovarian complex mass (7 x 6 x 5 cm) with two components, the main component is of high signal on T1WI/T2WI, attenuated on fat-saturated sequences (fatty component) and mural nodule (5 x 4.8 x 4.5 cm) of inhomogeneous high signal on both T1WI/T2WI with attenuation of the fatty component on fat-saturated sequences (Rokitansky nodule). No abnormal enhancement seen on postcontrast sequences.
Normal appearance of the left ovary.
Minimal effusion is seen in Douglas pouch.
