A 50 year old lady presented with a single episode of painless microscopic hematuria. As part of her workup, she underwent cross-sectional imaging which showed incidentally imaged situs inversus totalis and dextrocardia.

This case clearly illustrates this rare phenomenon. The patient had no symptoms arising from her situs inversus totalis, and proceeded to have an unremarkable hematuria workup.
