A partially circumscribed, irregularly shaped collection showing internal calcifications (representing spilled appendicoliths) and air foci with surrounding fat stranding which shows evidence of communication with an inflamed appendix in the right iliac fossa.

No lymphadenopathy is seen.

There is evidence of abundant fat in the pelvis with resultant stretched sigmoid colon and inverted pear-shaped urinary bladder.

The rest of the visualized viscera is unremarkable.

Case Discussion

CT findings are most likely suggestive of acute perforated appendicitis complicated by presacral abscess formation as described above.
