Syncopal event. Drinks one measure of spirits a day. Exercise tolerance of less than 20 meters.

Cardiac silhouette is grossly enlarged. Blunted costodiaphragmatic recesses. Lungs clear.

Case Discussion

A transthoracic echocardiogram showed an ejection fraction of 4%. The patient has since expressed a desire to stop drinking.
