Presentation
Twisting injury.
Patient Data

Moderate joint effusion.
Complete tear of the anterior cruciate ligament.
Linear intrasubstance increased fluid signal along with medial meniscus posterior horn not extending to meniscal periphery, favoring grade II horizontal tear.
Associated bone bruises/edema involving the middle weight-bearing part of lateral femoral condyle (lateral femoral notch sign), posterolateral and anteromedial tibial plateau, and fibular head.
Case Discussion
Complete anterior cruciate ligament (ACL) tear with lateral femoral notch sign as a result of an impaction injury of the femoral condyle and tibial plateau during rotational movement.
