Presentation

Radial artery thrombosis on external ultrasound. Reports previous manual labour therefore possible thenar hammer. Exclude proximal aneurysm or first rib compression.

CT angiogram of the left upper limb with the arm abducted over the head demonstrates high-grade extrinsic compression of the distal left subclavian artery as it passes between the first rib and scalenus anterior muscle. No accessory cervical rib is demonstrated. No aneurysm identified. The adjacent subclavian vein is not compressed.

The right subclavian and axillary arteries are normal noting the right arm is adducted.
