Presentation
Sudden onset of left-sided hemiplegia.
Patient Data

Acute infarction with water restriction involving the putamen, head, and body of the caudate nucleus as well as the anterior and posterior limbs of the internal capsule
High signal foci in T2 and flair sequences at subcortical and periventricular white matter of both cerebral hemispheres depicts microvascular ischaemic events.
Case Discussion
The basal ganglia supplied by medial and lateral lenticulostriate arteries. Medial lenticulostriate artery is a branch from the ACA that supplies the anteromedial head of the caudate and anterior limb of the internal capsule. Lateral lenticulostriate artery a branch from the MCA and supplies the putamen, head of the caudate, and posterior limb of the internal capsule. The infarction described in this case represents regions supplied by both arteries.
