Dilatation of the cystic duct stump (18 mm in diameter), containing a small stone (5 mm) with thick enhanced wall. 

Mild dilatation of the intra and extrahepatic biliary ducts with no CBD stone.

Case Discussion

MRI features most consistent with dilated cystic duct stump with stone and inflammation.

Prominent cystic duct stump is usually asymptomatic and discovered incidentally, however, the presence of inflammation and stones may cause symptoms (as in this case).
