Abnormal intrasubstance increased fluid signal and focal thickening is present along the course of the thumb flexor pollicis longus tendon at the level of zone TIII (from distal A1 pulley to carpal tunnel) related to tendinosis.
Case Discussion
Regarding the clinical and imaging findings, flexor pollicis longus tendinosis on the background of recent trauma is the most probable diagnosis.
The zone classification of flexor pollicis longus injuries divides into three zones based on anatomical location as followed 1:
- zone TI: from flexor pollicis longus insertion to A2 pulley
- zone TII: from A2 pulley to distal A1 pulley
- zone TIII: from distal A1 pulley to carpal tunnel
