Haemorrhagic brain metastases

Presentation

Headache. History of right breast cancer, post-surgical treatment.

Patient Data

Age: 40 years
Gender: Female

Multiple nodules are observed in the supratentorial and infratentorial brain parenchyma bilaterally, with the largest lesion located in the right temporal lobe, measuring approximately 17 × 16 mm. There is perilesional oedema, and the lesion exhibits heterogeneous signal intensity -predominantly hypointense on T1W, hyperintense on T2W and FLAIR, with mild diffusion restriction. Scattered hypointensities are noted on SWI with corresponding hyperintensity on Phase imaging (suggestive of haemorrhage). After contrast administration, the lesion demonstrates strong, heterogeneous enhancement.

Mucosal thickening of the paranasal sinuses. Bilateral fluid accumulation in the maxillary sinuses and mastoid sinuses.

An empty sella and a cavum vergae are also observed.

Imaging findings are consistent with multiple haemorrhagic brain metastases.
