Gradually worsening headache for several days. Otherwise well.

large cystic lesion in the posterior body of the right lateral ventricle
mild dilatation of the temporal and occipital horns of the right lateral ventricle
no contrast-enhancing lesions
Findings are consistent with an intraventricular simple cyst.
Case Discussion
"Intraventricular simple cyst" describes a cystic lesion in the cranial ventricles that follows CSF on all imaging modalities and phases/sequences. The lesion may be lined by arachnoid or ependymal cells, but this is indistinguishable on imaging. Patients are often asymptomatic at presentation, but hydrocephalus can occur.
