Hit by metal at work. On examination: tenderness, swelling, and abrasion left wrist and hand.

  • loss of Gilula arcs
  • proximal migration of capitate 
  • capitate not sitting within the distal articular 'cup' of the lunate
  • a line drawn through radius and lunate fails to intersect capitate 
  • lunate is seen displaced and angulated volar
  • soft tissue swelling.

There is volar dislocation of the lunate relative to the radius, with 90-degrees palmar rotation.
