Severe mitral regurgitation with large regurgitant jet within the left atrium. The left ventricle is moderately dilated with mildly reduced systolic function.

Case Discussion

Normally, only the right atrium abuts the right lung, forming an interface with the right middle lobe. If the left atrium enlarges, extends towards and also abuts the right lung, a second interface between heart (the left atrium) and lung forms: this is the double density sign. The cardiac MRI showed that the left atrial enlargement was due to severe mitral regurgitation.
