Status post mitral valve replacement with enlarged left atrial appendage and splaying of the carina due to enlargement of the left atrium. Lungs clear, no pleural effusion.

Case Discussion

Enlargement of the left atrial appendage results in a fourth "bump" along the left cardiomediastinal border: normal bumps include the aortic knob, the left pulmonary artery and the left ventricle. If enlarged, most commonly due to mitral valve disease, the left atrial appendage may form a fourth bump below the left pulmonary artery and above the left ventricle.
