Presentation
Gradual nasal obstruction and interrupted epistaxis for one year.
Patient Data

A well-defined large intensely enhancing right nasal mass was seen arising from the mid nasal septum with pedicle, associated with medial bowing of the nasal septum and deviation of the inferior nasal turbinate laterally. It shows free diffusion with high ADC. It measures 6 x 2.3 x 3.2 cm in AP x TR x CC respectively. right frontal, ethmoidal, maxillary and sphenoidal sinusitis.

The pathological report of the case confirms pyogenic granuloma with no atypia
Case Discussion
The patient underwent endoscopic sinus surgery and pathology confirmation of nasal pyogenic granuloma.
There is a nasal vascular mass arising from the mid-part of the nasal septum. It causes displacement of surrounding bone structures and free diffusion coping with its benign nature.
The main differential diagnosis includes:
- inflammatory sinonasal polyp
- inverted papilloma: arise from the lateral wall of the nasal cavity related to the middle nasal turbinate
- antrochoanal polyp: peripheral mucosal enhancement
- juvenile nasopharyngeal angiofibroma: exclusive in male patients and arising from sphenopalatine foramen
- other malignant nasal masses: older age, associated with bone destruction, and restricted on DWI
