Painless swelling of the right cervical region, progressively increasing in size with no fever. Laboratory investigations were normal.

There is a right multilocular cervical mass with internal septa, centred on the posterior cervical triangle with extension to the carotid and parotid spaces. it is of low signal with a component of high signal (protein content) on T1WI, high signal on T2WI. The postcontrast sequences show a peripheral rim enhancement of the component within the parotid space.
Case Discussion
MRI features suggestive of a nuchal cystic lymphangioma, also known as cystic hygroma.
