The nuchal ligament is a fibrous structure that extends from the external occipital protuberance to the spinous process of the C7 vertebra. The role of the nuchal ligament is to limit forward flexion and maintain the normal lordosis of the cervical spine.

Nuchal ligament ossification presents as a radio-opaque formation posterior to the cervical spinous processes. It is commonly discovered incidentally on lateral cervical radiographs.
