Ovarian torsion with a viable ovary

Presentation

Acute pelvic pain.

Patient Data

Age: 16 years
Gender: Female

The right ovary is enlarged showing oedematous stroma, peripheral follicles, and a corpus luteum. The ovary shows diffuse parenchymal enhancement. Swirling of the right adnexa is noted. The right ovary is abnormally located anteriorly and inferior to the uterus.

Normal size of the left ovary.

Mild pelvic ascites.

Case Discussion

The case shows an enlarged right ovary with massive ovarian oedema, peripheral follicles, and swirling of the adnexa, in keeping with ovarian torsion, the diffuse ovarian enhancement was suggestive of a viable ovary.

The patient was taken to the OR and underwent laparoscopic intervention which confirmed the diagnosis of ovarian torsion with a viable ovary which was treated by untwisting and oophoropexy.
