Presentation

Enlarging right tonsil for two weeks. Symptoms began as dysphagia. Given antibiotics as an outpatient but pain continued to worsen. Now presenting with difficulty eating and drinking and a muffled voice.

There is a low-attenuation collection along the lateral margin of the right palatine tonsil with peripheral enhancement. There is associated minimal effacement of the left parapharyngeal fat with minimal fat stranding throughout the right oropharynx.

There multiple enlarged, clustered, and/or hyperenhancing lymph nodes at the bilateral level IIa, IIb, III, IV, and Vb nodal stations, more so on the right side.

Case Discussion

This is a case of a peritonsillar abscess.

The patient was seen by otolaryngology who performed needle aspiration using an 18 gauge needle. The needle was advanced into the tonsillar fold until approximately 3 mL of purulent material was aspirated. The patient was subsequently provided oral antibiotics and followed up with otolaryngology as an outpatient. There was no evidence of recurrence.
