Mild internal rotation, loss of the half-moon overlap sign and the apparent small size of the humeral head on the AP radiograph are already suggestive of a posterior dislocation, which is directly confirmed by the Y view.
Case Discussion
In posterior shoulder dislocation the oblique view is not necessarily as revealing as in this case. Thus it is important to also keep the more subtle indirect radiographic signs in mind to avoid missing this diagnosis.
