Prepatellar bursitis

Diagnosis almost certain

Presentation

Anterior knee pain and swelling.

Patient Data

Age: 50 years
Gender: Female
mri

An effusion is present in the prepatellar bursa, anterior to the patella and patellar tendon, and displays low signal on T1 and bright signal on T2 and PD FS with some synovial proliferation of its walls.

Subcutaneous tissue edema of the anterior knee is also visible.

A well-defined heterogenous signal intensity lesion with internal low-signal areas (calcifications) in the distal femur and shows no marrow reaction suggestive of enchondroma.

Mild joint effusion.

Focal high signal intensity along with ACL anteroinferior fibers related to intrasubstance tear.

No other abnormalities within the joint structures.

Case Discussion

The case represents a distended fluid-filled prepatellar bursa secondary to chronic irritation and inflammation.
