Ruptured flexor pollicis longus tendon

Soft tissue injury in the thenar region of the right thumb, 10 days post-suture, with swelling and pain in the corresponding area. Restricted thumb movement.

MRI shows rupture of the right flexor pollicis longus tendon, with retracted tendon ends and associated oedema and fluid accumulation in the muscles of the corresponding thenar region. A previous surgical scar is also visible. No abnormalities are seen in the bones of the hand or fingers.

Case Discussion

The patient subsequently underwent surgery to confirm the diagnosis of complete rupture of the flexor pollicis longus tendon. Below is the surgical report:

  • a 4 cm incision was made along the old surgical scar in the thenar region of the right hand. Significant cloudy fluid and a moderate amount of necrotic tissue were observed beneath the skin. The flexor pollicis longus tendon was ruptured, along with the neurovascular bundle of the thumb

  • a debridement was performed, and the flexor pollicis longus tendon and the median nerve of the right thumb were exposed. Both tendon ends were identified, the nerve was partially fibrotic, and the surrounding tissue was clean

  • the decision was made to perform tendon and nerve repair using Prolen sutures 5/0 and 7/0

Post-surgery, the clinical symptoms improved.
