Presentation
Fall, lower back pain and hip pain, possibility of pelvic fracture
Patient Data

Bilateral inferior and superior pubic rami fractures, with minimal inferior displacement.
Diffuse generalized bone demineralization.

There are fractures of the inferior and superior pubic rami.
The superior pubic rami fractures extend into the anterior pillars of the acetabulum bilaterally.
There is no intra-articular extension and the hips otherwise appear intact.
There is also a fracture through the right side of the sacrum, adjacent to the sacroiliac joint.
There is a moderately severe compression fracture of T12.
There is biconcave endplate collapse of L2, L3, L4, and L5, which is likely non-acute.
Case Discussion
Straddle fracture is considered an unstable pelvic injury.
