Large subacromial spur with decreased space between the undersurface of the acromion and the humeral head, suggestive of a chronic rotator cuff tear.

Case Discussion

Subacromial impingement, where osteophytes arising from the inferior acromion or acromioclavicular joint damage the subjacent supraspinatus tendon, is a common mechanism of rotator cuff tear.
