Pain following strike in testicle by high-speed baseball. Assess for testicular hematoma versus rupture.

The testicle measures and is asymmetrically enlarged. The echotexture of the testis is heterogenous with multiple focal, avascular hypoechoic areas predominantly in the peripheral and mid to lower pole of the testicle, with regions of apparent discontinuity of the tunica albuginea. There is complex hydrocele with internal debris, likely representing a hematocele in the setting of trauma. There is otherwise normal arterial and venous blood flow to the remaining portions of the right testicle.

The left testicle is normal.

Case Discussion

This is a case of a ruptured right testis following trauma. The patient was seen by urology and nonoperative management was chosen.
