Presentation

Limping child for 3 months.

Patient Data

Age: 2 years
Gender: Female
x-ray

Mixed diaphyseal bone lesion centered on the cortex of the tibia, with a central osteolytic area surrounded by extensive sclerosis; note a discrete continuous lamellar periosteal reaction.

ct

Mixed medullary lesion with extensive medullary area of sclerosis, remodeling and cortical thickening, without rupture or invasion of adjacent soft tissues. Note the slightly spiculated pattern of the central lesion, denoting an area of osteogenesis.

Case Discussion

Due to the age group, an infectious cause was ruled out with laboratory blood tests. A CT guided bone biopsy was performed and the diagnosis of osteoblastoma was confirmed. Intralesional-type surgical resection and reconstruction with hydroxyapatite graft was performed.

Osteoblastoma is considered a benign bone-forming tumor, where there is no cellular atypia of osteoblasts. Unlike osteoid osteoma, it has a larger central area (nidus) usually greater than 2.0 cm.
