Complicated case of both infectious pancolitis and pyelonephritis with coalescing abscesses. The pattern of colitis is almost certainly infectious, particularly in the context of an acute presentation with clinical picture of infection. The presence of renal abscesses is a good reminder to avoid satisfaction of search and also that renal abscesses often present this way (mimicking cysts) as small collections which coalesce over time, and not typically as well-defined or enhancing collections.
