1. Do ultrasound first. Most of these lesions will turn out to be overtly benign, cysts or fibroadenomas. Work up is then complete. You limit unnecessary radiation by doing ultrasound first. You can always add the mammogram later, if needed.
2. The lesion should be well circumscribed with a thin pseudocapsule, broader than tall with minor lobulation if present ( less than 4) and homogeneous.
