Periventricular FLAIR hyperintensity adjacent to the frontal horns of the lateral ventricles represents ependymitis granularis. Small focus of FLAIR hyperintensity in the right superior frontal gyrus is in keeping with chronic small vessel ischaemia. Cavum septum pellucidum is also incidentally noted.
