Orbital abscess with fungal sinusitis

Discussion:

Features are suggestive of right orbital infection evidenced by right orbital cellulitis, right medial and inferior rectus myositis, and right optic neuritis complicated with right intraorbital abscess formation. It could be related to the ethmoidal, right frontal, bilateral sphenoid, and bilateral maxillary fungal sinusitis.

The patient underwent functional endoscopic sinus surgery (FESS), right medial meatal antrostomy and right orbital decompression.

Intraoperative findings showed mucopus in the right frontal, ethmoidal, right sphenoid and right maxillary sinuses. A fungal ball was seen in the right sphenoid sinus. Perforation of posterior nasal septum and right lamina papyracea defect with intraorbital pus is noted.
