Discussion:

Typical features of post-primary tuberculosis in an immunocompetent host: nodules, cavitation and bronchiectasis compatible with inflammation, destruction and fibrosis.

Endobronchial spread affects the left main and lower lobe bronchi with downstream opacities. This most likely accounts for the clinical deterioration.

The absence of hemoptysis makes acute hemorrhage unlikely.
