The system is divided into three categories: 

• A: total incongruity of the tarsometatarsal joint
• B: partial incongruity
  • B1 medial displacement of the first metatarsal 
  • B2 lateral displacement of the lesser metatarsals (this can be partial i.e. metatarsals 2-3, or complete i.e. metatarsals 4-5)
• C: divergent displacement
  • C1 partial 
  • C2 complete

This classification system is useful for fracture dislocations, however not so much for low energy Lisfranc injuries without fractures, low energy Lisfranc injuries also known as mid foot sprains should utilize the Nunley and Vertullo system.
