Large left lower lobe complicated cystic mass containing a crumpled internal membrane, characteristic for hydatid cyst with bronchial communication.  Hydatid disease in the lung is the second most common site after the liver.

Regarding the living in an endemic area of echinococcus granulosus, clinical and imaging findings, complicated pulmonary hydatid cyst is the final diagnosis.
