In the absence of history of trauma, the right scrotal fluid with a tendency to progressively expand, that is diffusely speckled with dense particulate matter visualized suggests chronic hydrocele. Diffuse debris noted may suggest infected hydrocele or scrotal cholesterolosis following cholesterol crystals depositions over time. Both B-grey mapping and 3/4D scrotal survey showed no gross solid tumors bilaterally. The left epididymal head appendix parenchymal calcification foci seen, portends calcified granulomas sequelae of chronic inflammatory process.
