Subarachnoid hemorrhage that is extensive and asymmetrically distributed should be treated as a ruptured aneurysm until proven otherwise. When no aneurysm is found following an extensive workup, sometimes with repeat CTA/DSA, we can consider perimesencephalic subarachnoid hemorrhage.
