The colon cut-off sign is formed by the inflammatory process extending from the inflamed pancreas into the phrenicocolic ligament via the transverse mesocolon and causing the abrupt interruption of the air column at the splenic flexure.

A laboratory test revealed the elevation of serum pancreatic enzymes and this patient was diagnosed with acute pancreatitis.
