Spigelian hernia is a rare type of abdominal hernia.

In our case, there was acute diverticulitis complicated with perforation, involving the sigmoid colon and its herniated segment.

However, there is another less likely possibility of perforation due to a complicated Spigelian hernia, despite its wide hernial neck.
