What is the differential diagnosis for this appearance?
The imaging differential diagnosis is between miliary tuberculosis and metastatic disease. TB is favoured given the history.
Innumerable tiny nodules are scattered throughout both lungs. In the left lower lobe anteriorly and laterally there are 2 regions of focal consolidation. There are multiple enlarged mediastinal lymph nodes. Hilar and axillary lymph nodes are not enlarged.
