A heterogeneous, unenhanced ovarian lesion above the level of the uterine should raise suspicion of ovarian torsion. The hyperdense areas suggest hemorrhagic zones.

This patient was immediately taken to the operating room, where the diagnosis of ovarian torsion was confirmed.
